Registrations for all three meetings will be processed at the AST National Office.

Registration Information
Register on or before Wednesday, January 31, 2007 to receive the pre-registration rates listed below. Registrations received on or after February 1 will be processed at the regular registration rates.
Please note: Membership status will be confirmed prior to registration being processed to verify whether you are a member in good standing.

Registration forms, including complete payment, must be received on or before January 31, 2007 to qualify for the pre-registration rates.
Three Ways to Register
Online
Register for any or all of the Winter Transplant Forum meetings online by clicking here . Links are also available from the ITNS website, www.itns.org, or the NATCO website, www.NATCO1.org. Registrations submitted online are not considered complete until processed. Online registration is payable with credit card only. A confirmation will be e-mailed to you prior to the meeting.
Fax
Download and print a registration form from any of the above websites. Complete the registration form, including complete contact information and credit card information, and fax the form to AST Registration at 856-439-9982.
Complete the registration form and include credit card information or a check payable to American Society of Transplantation for your registration fee. For express delivery, use telephone number 856-439-9986.

CONFIRMATION OF REGISTRATION
Confirmation of your registration will be emailed to you prior to the meeting. Your badge and other meeting materials will be available for pick up when you arrive at the meeting.
PAYMENT
Acceptable payment forms include: checks made payable to the American Society of Transplantation (drawn on a U.S. bank in U.S. dollars), VISA, MasterCard, and American Express. Full payment must accompany all registration forms. Registration forms without accompanying full payment will be returned for completion. Registrations will not be processed without payment.
CANCELLATIONS/REFUNDS
To cancel your registration and receive a refund, a written request must be received in the AST office on or before January 31, 2007. Cancellation requests received by this date will be refunded less a $25 processing fee. Requests for refunds will be processed after the meeting. There will be no refunds issued for requests received on or after February 1, 2007.
